Here we have a Sage Green Recycled Heavy Thick Square Glass Candle Holder Fruit Votive 3" Tall. I would rate it to be in a (8/8+) lightly used working shape with very little normal use marks. Embossed with grapes, apple, pear, cherries. It stands 3" tall and 2 3/4" wide. It has lots of air bubbles in the glass which is consistent with recycled glass.
